RESUMO

Acute generalized pustular psoriasis (GPP) is a serious illness. Despite various treatment methods, there is still lack of effective treatment plans for refractory cases with multiple comorbidities. This case report presents a 67-year-old woman with acute GPP, stage 4 chronic kidney disease (CKD), type 2 diabetes, and cardiovascular disease, in whom skin symptom disappearance and kidney function improvement were observed after the use of oral tacrolimus as the sole therapy. This is the first report on the application of tacrolimus in the treatment of acute GPP, especially refractory acute GPP. The successful treatment indicates that there are shared immune pathways between acute GPP and CKD, and the pathways can be interdicted by tacrolimus. Further studies are needed to optimize the therapy to maximize efficacy and minimize toxicity.

OBJECTIVE: Analyze the research state and development trend of photodynamic therapy for dermatology using visual knowledge graphs derived from the Web of Science Core Collection database. METHODS: The Web of Science Core Collection database was utilized as the search data source for the bibliometric analysis, and the associated articles published between January 1, 2000, and December 31, 2022, were obtained using the search terms "photodynamic therapy" and "dermatology". CiteSpace, VOSviewer, and additional tools were utilized for bibliometric analysis, and visual knowledge graphs were created. RESULTS: Eight hundred and thirty two articles were retrieved in total, and 747 were included following de-duplication and transformation. The country with the greatest number of publications is the United States; the primary research institution was University of Copenhagen; and the references with the highest centrality were primarily concerned with the selection of photosensitizers; High frequency keywords primarily comprised 5 aminolevulinic acid and basal cell carcinoma; and the clustering graph revealed that all keywords fell into 11 categories. CONCLUSION: In numerous areas of dermatology, photodynamic treatment is commonly employed. Current research focuses on nonneoplastic skin diseases and the choice of photosensitizers. Nonetheless, its specific mechanism and other applications merit further investigation.

Mycobacterium chelonae is a non-tuberculous mycobacteria, which can cause skin infectious granuloma through cosmetic injection. This disease's treatment requires a combination of sensitive antibiotics and a lengthy treatment cycle. Photodynamic therapy is still effective for patients who are unwilling to take antibiotics orally. In this case, we successfully used fire needle combined with photodynamic therapy to treat skin infectious granulomatosis caused by Mycobacterium chelonei, and achieved satisfactory results. We used fire needle to pretreat cysts and nodules, which improved the diffusion and absorption of locally applied photosensitizers and enhanced the efficacy of photodynamic therapy. However, additional clinical research is necessary to determine the efficacy and safety of fire needle combined with photodynamic therapy for cutaneous infectious granulomatosis.

Recently, employing metal-organic frameworks (MOFs) as precursors to prepare various metal oxides, sulfides, and selenides has drawn enormous attention in the field of energy storage. In this paper, the nanosheets of an organophosphate-based Ni-MOF were successfully synthesized and employed as the template to prepare the Prussian blue analogue (PBA) nanoslices and nanoparticles on the nanosheet (PBA/Ni-MOF-NS-x h, x h stands for the reaction time.) by an in situ etching method. After selenization by the solvothermal method, the PBA nanoslices and nanoparticles were transformed into spongelike bimetallic selenides (labeled as PBA/Ni-MOF-NS-x h-Se) decorated with some nanoparticles. All of the characterization results including PXRD, SEM, TEM, EDS, XPS, and BET demonstrated the successful transformation. Impressively, the as-synthesized PBA/Ni-MOF-NS-12 h-Se exhibited a high specific capacitance of 1897.90 F g-1 at a current density of 1 A g-1 and a superior capacitance retention rate of 73.32% as the current density increased to 20 A g-1. In addition, the asymmetric supercapacitor device, PBA/Ni-MOF-NS-12 h-Se//AC, delivered a high energy density of 30.69 W h kg-1 at 0.85 kW kg-1 and extraordinary cycling stability with an 83.00% capacitance retention rate over 5000 cycles.

High-performance water splitting electrocatalysts are urgently needed in the face of the environmental degradation and energy crisis. The first principles method was used in this study to systematically examine the electronic characteristics of transition metal (Sc, Ti, V, Cr, Mn, Fe, and Ru) doped WSi2N4(TM@WSi2N4) and its potential as oxygen evolution reaction (OER) catalysts. Our study shows that the doping of TM atoms significantly improves the catalytic performance of TM@WSi2N4, especially Fe@WSi2N4shows a low overpotential (ηOER= 470 mV). Interestingly, we found that integrated-crystal orbital Hamilton population and d-band center can be used as descriptors to explain the high catalytic activity of Fe@WSi2N4. Subsequently, Fe@WSi2N4exhibits the best hydrogen evolution reaction (HER) activity with a universal overpotential of 47 mV on N1sites. According to our research, Fe@WSi2N4offers a promising substitute for precious metals as a catalyst for overall water splitting with low OER and HER overpotentials.

BACKGROUND: Although the 308 nm excimer laser is commonly used to cure vitiligo, its clinical efficacy is limited. Experts have found that the efficacy of compound glycyrrhizin tablets combined with the 308 nm excimer laser in the treatment of vitiligo is significantly enhanced, but the specific research methods and clinical data must be clarified. OBJECTIVE: To determine the clinical efficacy of compound glycyrrhizin tablets combined with the 308 nm excimer laser in the treatment of vitiligo. METHODS: Until August 2022, studies were searched in PubMed, the Cochrane Library, the Chinese Biomedical Literature Database, the China National Knowledge Infrastructure, the Chinese Scientific Journal Database, and the Wan Fang Database. We also searched for clinical RCTs involving compound glycyrrhizin tablets combined with the 308 nm excimer laser for the treatment of vitiligo. The meta-analysis was conducted in accordance with the Cochrane Handbook's recommendations. Two reviewers regulated the study selection, data extraction, and assessment of bias risk, and consulted a third reviewer as necessary. For the meta-analysis, Review Manager 5.4 was utilized. RESULTS: Finally, 9 articles and 1052 patients were included. A systematic review compared the efficacy of compound glycyrrhizin tablets combined with the 308 nm excimer laser to that of the 308 nm excimer laser alone: OR = 3.33, p < 0.00001, 95% confidence interval [2.25, 4.92]. CONCLUSION: In the treatment of vitiligo, compound glycyrrhizin tablets combined with the 308 nm excimer laser are more effective than the 308 nm excimer laser alone, and there are no serious adverse reactions. It is a safe and efficient way of treatment.

Two-dimensional (2D) MoSi2N4is a newly created material that has superstability and ultrahigh carrier mobility. Besides, the hydrogen evolution reaction activity was proved excellent by doping transition metal (TM) atoms and introducing N vacancies. But, the oxygen reduction reaction (ORR) and oxygen evolution reaction (OER) of 2D MoSi2N4is unclear even. We have explored the electrocatalytic properties (OER/ORR) of MoSi2N4by introducing Si vacancies and attaching various TM atoms. The structure and optoelectronic characteristics of MoSi2N4have been researched in detail using density functional theory calculations. By analyzing the density of states, the free energy change diagram and contour maps of TM@VSi-MoSiN, the results show that Co@VSi-MoSiN has the lowest OER overpotential (0.53 V) among all samples. Additionally, the d-band center is used to explain the electrocatalytic origin of the OER and ORR of TM@VSi-MoSiN. Our discoveries expand the 2D TM@VSi-MoSiN applicability in the realm of catalysis.

In this study, we investigate a stochastic predator-prey model with anti-predator behavior. We first analyze the noise-induced transition from a coexistence state to the prey-only equilibrium by using the stochastic sensitive function technique. The critical noise intensity for the occurrence of state switching is estimated by constructing confidence ellipses and confidence bands, respectively, for the coexistence the equilibrium and limit cycle. We then study how to suppress the noise-induced transition by using two different feedback control methods to stabilize the biomass at the attraction region of the coexistence equilibrium and the coexistence limit cycle, respectively. Our research indicates that compared with the prey population, the predators appear more vulnerable and prone to extinction in the presence of environmental noise, but it can be prevented by taking some appropriate feedback control strategies.

BACKGROUND: Moderate-to-severe acne affects people's health and quality of life. As first-line therapeutic medications, isotretinoin and antibiotics are used to treat moderate-to-severe acne, but outcomes can be improved. The combination of fire needle and ALA-PDT may be one option. This study evaluated the safety and effectiveness in the treatment of moderate-to-severe acne by the combination with fire needle and ALA-PDT. METHODS: By July 2022, search PubMed, the Chinese Biomedical Literature dababase, the Cochrane Library, the Chinese Scientific Journal Database, the China National Knowledge Infrastructure, the Web of Science Datebase, Embase Datebase,VIP Database and WanFang Database. To gather RCTs of the combination of fire needle and ALA-PDT for the treatment of moderate-to-severe acne. A meta-analysis was performed according to the Handbook guidelines of Cochrane. Study selection, data extraction, and risk of bias evaluation were all governed by two reviewers, with the help of a third reviewer if necessary. The meta-analysis was carried out with Review Manager Software 5.4. RESULTS: There were a total of 9 RCTs with 862 participants. Clinical efficacy was recorded in nine trials, GAGS score was published in three studies, adverse events were documented in five studies, and the recurrence rate was reported in two studies. Treatment lasted between four and twelve weeks. Combination therapy outperformed monotherapy in terms of clinical efficacy (OR:3.73; 95% CI:2.51, 5.53; p < 0.00001). Additional subgroup analysis revealed that the combination therapy outperformed ALA-PDT alone in terms of clinical effect (OR: 3.20; 95% CI: 2.05, 4.99; p < 0.00001). Additionally, combination therapy outperformed fire needle alone in terms of clinical efficacy (OR:5.66; 95% CI: 2.66, 12.08; p < 0.00001). Studies have also indicated that combination therapy has a stronger benefit in lowering the GAGS score (MD:-3.35; 95% CI:-4.62, -2.09; p < 0.00001). Additionally, there was no discernible difference in the occurrence of adverse events between the combined treatments and monotherapy (OR:1.43; 95% CI: 0.76, 2.69; p = 0.26), and the combined treatment was able to control the recurrence rate (OR:0.18; 95% CI: 0.07, 0.45; P = 0.0002). CONCLUSIONS: The efficacy of fire needle combined with ALA-PDT in the treatment of moderate-to-severe acne appears superior to that of ALA-PDT or fire needle alone. However, the conclusions of this study must be interpreted carefully due to the high risk and ambiguity of bias of the included trials.

Capsular contracture caused by silicone rubber is a critical issue in plastic surgery that urgently needs to be solved. Studies have shown that carbon ion implant in silicone rubber (carbon silicone rubber, C-SR) can significantly improve the capsular structure, but the effect of this improvement only appear 2months or later. In this study, asiaticoside combined with carbon silicone rubber was used to explore the changes in the capsule to provide a reference for the treatment of capsule contracture. Human fibroblasts (HFF-1) were used for in vitro experiments. The combined effect of asiaticoside and carbon silicone rubber on cell proliferation was determined by the CCK8 method, cell migration changes were measured by Transwell assays, cell cycle changes were measured by flow cytometry, and the expression levels of fibroblast transformation markers (vimentin and α-SMA), collagen (Col-1A1) and TGF-ß/Smad signaling pathway-related proteins (TGF-ß1, TßRI, TßRII and Smad2/3) were detected by immunofluorescence. In vivo experiments were carried out by subcutaneous implantation of the material in SD rats, and asiaticoside was oral administered simultaneously. WB and ELISA were used to detect changes in the expression of TGF-ß/Smad signaling pathway-related proteins. TGF-ß/Smad signaling pathway proteins were then detected and confirmed by HE, Masson and immunohistochemical staining. The results shown that asiaticoside combined with carbon ion implantation inhibited the viability, proliferation and migration of fibroblasts on silicone rubber. In vitro immunofluorescence showed that the secretion levels of α-SMA and Col-1A1 were significantly decreased, the transformation of fibroblasts into myofibroblasts was weakened, and the TGF-ß/Smad signaling pathway was inhibited. In vivo experimental results showed that asiaticoside combined with carbon silicone rubber inhibited TGF-ß1 secretion and inhibited the TGF-ß/Smad signaling pathway, reducing the thickness of the capsule and collagen deposition. These results imply that carbon silicone rubber combined with asiaticoside can regulate the viability, proliferation and migration of fibroblasts by inhibiting the TGF-ß/Smad signaling pathway and reduce capsule thickness and collagen deposition, which greatly reduces the incidence of capsule contracture.

Background: Facial seborrheic dermatitis (FSD), also called facial seborrheic eczema, is a common disease affecting both male and female patients worldwide. Tanshinone is the main bioactive component extracted from the Traditional Chinese Medicine Salvia miltiorrhiza Bunge, which is widely used in treating skin inflammatory diseases. It is necessary to evaluate the clinical evidence for tanshinone capsule treatment of FSD. This study aimed to evaluate the safety and effectiveness of tanshinone capsules combined with prednisone in the treatment of facial seborrheic dermatitis and to provide evidence for clinical practice. Methods: Studies were searched in PubMed, the Cochrane Library, the Chinese Biomedical Literature Database, the China National Knowledge Infrastructure, the Chinese Scientific Journal Database, and WanFang Database before October 2021. We also searched for randomized controlled clinical trials (RCT) of tanshinone capsules combined with prednisone on facial seborrheic dermatitis. The meta-analysis was conducted according to the guidelines of the Cochrane Handbook. Two reviewers regulated the research selection, data extraction, and risk of bias assessment, respectively, and a third reviewer was used for consulting when necessary. Review Manager Software 5.3 was used for meta-analysis. Results: A total of 10 RCTs with 916 participants were included. Nine studies reported total effectiveness, five studies reported symptom score, seven studies reported adverse events, and four studies reported recurrence rate. The duration of treatment was 4 to 8 weeks. Combination therapy showed better clinical effects compared to the prednisone (OR: 5.82; 95% CI: 3.53, 9.59; p < 0.00001). Combination therapy could repair skin lesions (MD: -0.40; 95% CI: -0.51, -0.30; p < 0.00001), reduce skin erythema (MD: -0.58, 95% CI: -0.67, -0.49; p < 0.00001), relieve skin itch (MD: -0.70; 95% CI -0.77, -0.63; p < 0.00001), and desquamation score (MD: -0.64; 95% CI: -0.71, -0.56; p < 0.00001). Furthermore, combination therapy could reduce adverse events (OR: 0.46; 95% CI: 0.26, 0.84; p = 0.01) and control recurrence rate (OR: 0.22; 95% CI: 0.13, 0.36; p < 0.00001). Conclusions: Compared with prednisone, tanshinone capsules combined with prednisone may be effective in the treatment of facial seborrheic dermatitis. However, due to the high risk and ambiguity of bias in the included trials, the conclusion of this study must be interpreted carefully.

Vitiligo is a common and refractory disease worldwide. The limited efficiency and side effects of the conventional treatment options create demands towards the development of strategies. Excellent repigmentation is demonstrated after several filiform fire needle sessions in the vitiligo lesions. In this observational study, we aimed to observe the response to filiform fire needle therapy in patients with vitiligo, and determine whether there was a difference of efficiency with respect to the type, affected site, and disease duration of vitiligo. Patients received filiform fire needle therapy once every 2 weeks for 12 consecutive weeks. The results of the 77 vitiligo patients were: 34 (44.15%) with an excellent repigmentation rate, 15 (19.48%) with a marked improvement, 15 (19.48%) with a moderate response, 6 (7.79%) with a slight improvement, and 7 (9.09%) with an absent response. Among the vitiligo patients with different affected sites, the most effective location of therapy was the face. Shorter course leads to better therapeutic effect. Two patients developed hypertrophic scars on the lesion site. In conclusion, this study shows filiform fire needle therapy is an effective and relatively safe therapeutic option for vitiligo.

Sporothrix species are commonly isolated from environmental and clinical samples. As common causes of zoonotic mycosis, Sporothrix species may result in localized or disseminated infections, posing considerable threat to animal and human health. However, the pathogenic profiles of different Sporothrix species varied, in virulence, geographic location and host ranges, which have yet to be explored. Analysing the genomes of Sporothrix species are useful for understanding their pathogenicity. In this study, we analyzed the whole genome of 12 Sporothrix species and six S. globosa isolates from different clinical samples in China. By combining comparative analyses with Kyoto Encyclopedia of Genes and Genomes (KEGG), Carbohydrate-Active enZymes (CAZy), antiSMASH, Pfam, and PHI annotations, Sporothrix species showed exuberant primary and secondary metabolism processes. The genome sizes of four main clinical species, i.e., S. brasiliensis, S. schenckii, S. globosa, and S. luriei were significantly smaller than other environmental and clinical Sporothrix species. The contracted genes included mostly CAZymes and peptidases genes that were usually associated with the decay of plants, as well as the genes that were associated with the loss of pathogenicity and the reduced virulence. Our results could, to some extent, explain a habitat shift of Sporothrix species from a saprobic life in plant materials to a pathogenic life in mammals and the increased pathogenicity during the evolution. Gene clusters of melanin and clavaric acid were identified in this study, which improved our understanding on their pathogenicity and possible antitumor effects. Moreover, our analyses revealed no significant genomic variations among different clinical isolates of S. globosa from different regions in China.

RATIONALE, AIMS AND OBJECTIVES: The purpose of this study is to systematically evaluate the quality of methodological guidelines for androgenetic alopecia (AGA) using the Appraisal of Guidelines for Research and Evaluation (AGREE) II instrument by searching and analysing the available worldwide guidelines. This could provide a reference for selecting clinical guidelines and for developing new guidelines in the future. METHODS: We searched PubMed and other electronic databases for any AGA guidelines published until 2014. The AGREE II instrument was used by two researchers to create a systematic appraisal in six domains to determine the guidelines fulfilling the inclusion and exclusion criteria. RESULTS: Five guidelines associated with the therapy of AGA were identified. The mean scores for the six AGREE II domains were as follows: 89.45% for 'scope and purpose', 53.88% for 'stakeholder involvement', 53.96% for 'rigour of development', 87.22% for 'clarity of presentation', 42.92% for 'applicability' and 59.16% for 'editorial independence'. The European guideline ranked the highest, whereas the American guideline of 1996 ranked the lowest. The Asian, European and Japanese guidelines were strongly recommended, and the two from America were recommended with modifications. CONCLUSIONS: Guidelines should provide accurate and evidence-based recommendations. The AGREE II instrument is a useful tool to improve the quality of guidelines, and high-quality guidelines for clinical practice can be provided by using it.
